Investigations are underway to establish the cause of the sinking of the Fraserburgh-based trawler Ocean Way some 100 miles east of the Farne Islands in the North Sea on Sunday, 2 November, but already a sadly familiar issue has already surfaced: lifejackets were not worn.

Search and rescue efforts to locate two missing crewmembers have been suspended. Two survivors and the body of the deceased skipper were pulled from the water. Neither survivor, nor the body of the skipper wore lifejackets.

A vessel can come to grief with extraordinary swiftness. There may be little time to launch a liferaft or to find and don lifejackets. Without a flotation device the extra effort needed to keep afloat encourages the onset of hypothermia and prevents seafarers helping each other. Continue reading »

On 7 June, 2014 a worker fell about four metres from the lower landing of an
offshore platform into the sea. He was retrieved from the water without delay by
the crew on a support transfer vessel and transferred to the site clinic for medical treatment.

The  Type 1 Inflatable Personal Flotation Device, PFD. the worker was wearing did not automatically inflate on contact with water despite being equipped with this capability. Inspection of the PFD after the incident identified that the gas canister was not fully screwed into the inflation tube fitting. Continue reading »

dp2Single fault failures should not be possible in safety critical systems. However, a recent incident in which dynamic positioning failed while divers were underwater show that they can and do happen in ways that, with 20/20 hindsight, are not surprising.

A serious incident occurred in which a diving support vessel’s dynamic positioning (DP) system, designated as IMO class 2, failed resulting in the vessel drifting off position while divers were deployed subsea. Investigations have shown that a probable cause of the DP failure was a single fault which caused blocking of the DP system’s internal data communications. Continue reading »

If you carry Mullion Compact 150N likejackets check whether the oral inflation tube has a yellow plastic insert. If so, it should be removed from service and returned to Mullion says an alert from the Marine Safety Forum.

Mullion has reported that there is a potential Safety concern with the Compact 150N ISO 12402 Lifejackets, once it is inflated. Mullion has recalled all Compact Lifejackets which have been sold since 1st Dec 2011 if they have a YELLOW valve in the oral
inflation tube.

Compact Lifejackets with a RED valve in the oral inflation tube are not subject to this recall.

The affected lifejackets are being recalled as a precaution for inspection and will either be repaired or replaced.

Advises MSF: “All vessels are requested to inspect the Inflatable Lifejackets carried onboard. Vessel’s that are
equipped with the make and model of Inflatable Lifejackets as described in this Safety Flash
should:
1. REMOVE THE LIFEJACKETS FROM SERVICE WITH IMMEDIATE EFFECT
2. CONTACT the QHSE department to arrange landing/exchange”

US Coast Guard says it has become aware of certain Mustang Survival Inflatable PFDs with Hammar MA1 hydrostatic (HIT) inflation systems which may not inflate and require a new re-arm kit to properly inflate by manual or automatic activation. It has issued a safety alert which identifies which products are affected.

Certain inflatable PDFs may be subject to delayed or non-inflations. To determine if you are affected please follow these instructions:

Some models of personal flotation devices produced by Mustang Survival have been found to have an inflator installation inconsistency that may prevent some units from fully inflating with CO2 and are being recalled. The affected models are MD2010 and MD2012 model 22LB sold in the US in 2011. Alerts have been issued by the US Coast Guard and by Mustang Survival which had issued a voluntary recall notice.

Mustang products with white labels are not affected by the recall. Similarly, MD2010 or MD2012 PFDs with an “MIT” stamp are useable and do not need to be returned.

AnyMD2010/MD2012 missing the “MIT” stamp should be returned to Mustang

Mustang Survival has developed a solution that corrects any affected product and prevents re-occurrence of this issue. The inspection and repair can only be performed at a Mustang Survival factory.

Inflatable lifejacket - You're mad not to wear one

Lifejackets are useless unless worn. It’s a simple lesson that applies to merchant mariners and fishermen. It’ a lesson still not getting across.

Britain’s Maritime & Coastguard Agency says that 86 maritime deaths between 2007 and 2010 may have been avoided if those involved had worn a lifejacket or buoyancy aid.

This is the latest finding of an expert panel which has been analysing maritime accidents since 2007. Recently the panel, consisting of members from the MCA, RNLI, RYA, MAIB1, lifejacket industry and Portsmouth University, looked at the 35 maritime deaths in 2010 where lifejacket wear may have been appropriate. They agreed that in 21 of those incidents a life might have been saved if a lifejacket or buoyancy aid had been worn. Continue reading »

image Some lifejacket cheststraps may present a hazard in an emergency, warns the US Coastguard.

Recent Coast Guard inspections of Type I Personal Flotation Devices, (PFDs) in both adult and child size, identified a potential hazard that could prevent proper donning in the event of an emergency. The chest strap was threaded through the fixed “D” ring that the strap is intended to clip to when worn. (Image left)
It was discovered that several PFDs were assembled this way at the factory and if not corrected could create a hazardous condition during an emergency when they are donned.
Instead of the strap falling away, allowing the wearer to wrap it around him or her, the clip end of the strap could snag in the “D” ring preventing the wearer from getting it around their body. (Following image)
