There’s a good reason why NWEA Guidelines Section 4.1 says: ““Avoid use of heavy sections of reducers or connections at hose ends” and here it is:
“Whilst in the process of connecting a bulk fuel hose to the vessels manifold an AB received a knock to his elbow when the hose connection dropped to deck.
Onboard investigation found that an additional 4 inch Weco connection was
in place to enable the Avery Hardol to be connected. This additional connection increased the weight and led to manual handling difficulties for the vessels crew”.
