In an unprecedented move Maersk has released CCTV footage of an engineroom fire aboard Maersk Iowa to help determine the cause of the fire. The footage was posted on the gCaptain site.

A statement from Maersk, published by gCaptain says: “The video footage (posted on gCaptain today) depicts the mechanical failure of a main engine air start valve resulting in an explosion and fire in the engine room of the Maersk Iowa while underway on January 10th, 2015. No one was injured by this incident, and thanks to the quick and professional emergency response of the officers and crew, any potential further damage was contained with no harm to the environment. The Company has shared this particular video footage with the USCG, Lloyd’s Register and other internal and external stakeholders in an effort to understand its root cause for implementing corrective action across our fleet. The video has also been shared with our ships crews’ as a training aid.”

Given the enthusiasm displayed by lifeboats to fall off their hooks with depressing regularity one would hope that fitting a fall prevention device, FPD, to a lifeboat during drills is regarded as good seamanship these days. On the other hand one that is not properly arranged is not going to do its job, as a safety alert from Marine Safety Forum, MSF, explains.

An MSF member reports that during hoisting of a starboard lifeboat, it reached the upper deck, it was noticed that the FPD was not properly secured or attached to the lifeboat. Continue reading »

MAC has previously posted on the issue of weighted monkeyfists but as the Facebook page of the Maritime Confidential Hazardous Incident Reporting Programme, CHIRP, shows, the problem is no going away. Indeed, the photo reproduced here courtesy Robert Wilkinson, indicates that the word isn’t getting through – two of them come from the same vessel even after a complaint was made.

The knot is intended to give mass to the end of a line to improve both reach and accuracy when throwing a line. Getting hit by lumps of metal wrapped in a monkeyfist is dangerous, and Britannia P&I Club and West of England P&I Club have issued warnings following incidents, including ones resulting in injuries. …Not yet, except in the imagination of researchers at Tehran-based RTS Ideas which had developed and claims to have tested a search and rescue drone in the Caspian Sea. Equipped with deployable lifebouys, cameras and infra-red sensors for night time use, the drone in development appears to be targetted at coastal areas but shipboard and offshore utilisation could become possible.

Presently the device has a range of 4.5 kilometres and is operated by a lifeguard using radio control.

Says the company: “Pars is an Aerial robot designed and made for saving human lives. The first purpose of building the robot is the relief of people drowning near coastlines. By developing its applications, it can be used in ships and off shore reliefs. It can also be used in other applications such as monitoring of marine and off shore structures, recording films and pictures from dangerous path ways for rescue missions, precise positioning. One of the features of this robot is Ability to save more than one life in a mission it can also track its path by GPS positioning and at the end of its mission it can come back home without the need of user guidance.”

A still from Lifejacket: A Fisherman’s Friend

Two Bridlington fishermen have made a short film to encourage their fellow fishermen to wear lifejackets whilst at sea. The two minute film, entitled “Lifejacket: a fisherman’s friend” is available on Youtube and may be watched below.

The decision to make the film came after a panel of experts* concluded that 26 fishermen could still be alive today had they been wearing a lifejacket when they were involved in an incident at sea (2007-11 figures). The Fishing Industry Safety Group (FISG), were so alarmed by the new statistic that they put the idea of a short film forward to fishermen Dylan Silverwood and Christopher Stewart.
